“The PRM Musculoskeletal Ultrasound Interventional Regenerative Movement:
Championing Local to Global Physiatric Practice”

Musculoskeletal ultrasound (MSKUS) has evolved tremendously, from its first report published in 1958 by K. T. Dussik, who measured the acoustic attenuation of articular and periarticular tissues including skin, adipose tissue, muscle, tendon, articular capsule, articular cartilage and bone. 1 This work led to many others as improvements in technology, computer processing, and applications, among many, expanded the application of ultrasound use. Today, it has become an adjunct assessment to a more evidence-based point of care, musculoskeletal ultrasound management has become a natural adjunct to the physician’s clinical management.
Physical Medicine and Rehabilitation’s natural evolution to include interventional procedures was inevitable. It is with this recognition that the Philippine Academy of Rehabilitation Medicine National Capital Region (PARM-NCR chapter) took initiative to provide extensive training in MSKUS through its internationally certified Registered Musculoskeletal Ultrasound advisers and faculty. For two years now, in collaboration with SMARTMD, the PARM NCR chapter has spearheaded BURST– Basic Ultrasound Rehabilitation Specialty Training Workshops for the shoulder, elbow, wrist and hand, knee, and finally the hip. These workshops were distributed throughout the year in 2023, which garnered at least 5 CPD units per workshop. Anatomy of the specific body part is emphasized in each workshop, followed by the proper scanning protocol and the ultrasound evaluation. Clinical correlation is done, relevant to the practice of PRM, and finally, live, interactive MSKUS guided interventional, regenerative procedures are demonstrated with live patients to give attendees confidence in doing the procedures from diagnosis to management. The attendees are given time to break out into groups with RMSKUS certified faculty in ratios of 5 attendees to 1 faculty. This allows each attendee to be guided while scanning.

Aspire to inspire. No one better to inspire practicing Filipino physiatrists than the leading author of the book Ultrasound of the Musculoskeletal System and world-renowned authority in musculoskeletal ultrasound than the Prof. Dr Carlo Martinoli. The PARM NCR chapter in cooperation with SMARTMD and the Philippine Academy of Rehabilitation Medicine collaborated to bring for the first time in the Philippines, MASTERS by Martinoli. This 7-day workshop of unparalleled in-depth musculoskeletal ultrasound training from October 27 to November 2, 2023 was a historical event which raised the PRM level of MSKUS awareness and expertise, in a 3-day peripheral nerves workshop course, followed by a 4-day upper extremity workshop and course. The top-notch caliber faculty included Dr Martinoli’s chosen team of experts from Italy, Netherlands, Australia, Taiwan, Hongkong and our very own Philippines pride, Dr Franklin SP, Domingo and PARM NCR chapter scientific committee adviser, Dr Jeimylo C. De Castro. PMR Residents in training were given the unique opportunity to serve as models with open-access to the high yield course, attended by different specializations from different countries from the Asia- Oceanea, Europe and the Americas. Filipino PMR associates, diplomates and fellows participated eagerly, with the NCR chapter forming the bulk of the local PRM attendees. This solidifies the PARM NCR commitment to bring the practice of regenerative interventional musculoskeletal ultrasound at par with the best of the world, and bring PARM awareness to the world.

January 28, 2024, opened the year with BLAST– Bridging Learning in Advanced Sonography Training: Lumbosacral Spine, at the Westin Manila. From basic training, the PARM NCR chapter offer advanced sonographic training as a natural progression of increasing knowledge and application of learning. Following the same program as the BURST series with more challenging cases and procedures, the BLAST series will continue to offer continuing medical education to encourage more physiatrists to improve skills level and quality of patient management. This will be an ongoing program with the PARM NCR chapter’s goal to “aspire to inspire” and “inspire with desire”. Afterall, “You only see what you know”. MSKUS is a definite game-changer in the management of our patients. It is our responsibility as physiatrists to ASK what we can do for our patients, continue to SEEK new management that will allow patients more options to improve their quality of life, as our patients KNOCK on our doors to be the instruments of healing. With the clear vision of elevating the practice of physiatrists in the Philippines, PARM NCR chapter stays true to its aspirations to BEGIN WITH THE END IN MIND. By focusing on the PARM NCR goal, grey zones are as clear as black and white. Knowing our strengths, overcoming our weaknesses and aiming for what we desire the most with the chapter moving in unison gives the needed momentum and driving force which propels the chapter, and the academy as a whole, in this global phenomenon which has changed the way musculoskeletal medicine has evolved. Championing local to global physiatric practice through high calibre continuing medical education and workshops, PARM NCR chapter has made a commitment to make this subspecialty a resounding and undeniable feature in the evolving PRM specialization. Musculoskeletal Interventional Regenerative management should be a unique niche in the PRM armamentarium. It is worth protecting and fighting for. With this, all efforts should be focused on making the right noise echo in the medical field.

From allowing more accurate assessment to a more evidence-based point of care, musculoskeletal ultrasound management has become a natural adjunct to the physician’s clinical management.

Altogether, interactive MSKUS-guided, interventional, regenerative procedures were demonstrated with live patients to enhance the confidence of the participants in performing the procedures from diagnosis to management. The participants were divided into break-out groups, with RMSKUS- certified faculty as facilitators per group, guiding the participants step-by-step.
