“You only see what you know:
Black, White, and 50-Plus Shades of Grey”

Paschenelle B. Celis, MD, FPARM
On February 25, 2024, the first musculoskeletal ultrasound post-conference skills laboratory workshop was conducted. Led by a roster of Registered Musculoskeletal Ultrasound (RMSK) certified faculty by the Alliance for Physician Certification and Advancement (APCA), the workshop focused on high-yield topics on the shoulder, wrist and hand. The program included lectures on anatomy of the shoulder, discussed by Rochelle Angelica Saratan, MD, FPARM, RMSK, while the scanning protocol was handled by Franklin Domingo, MD, FPARM, RMSK. The anatomy of the wrist and hand was presented by Ronald Christoher Cua, MD, FPARM, RMSK. This was followed by a discussion of the scanning protocol by Michael Francis Obispo, MD, FPARM, RMSKJeimylo De Castro, MD, FPARM, RMSK, DABRM, CIPS conducted the live patient demonstration of MSKUS-guided orthobiologic injection of the shoulder addressing a diagnosed case of chronic supraspinatus tendinosis. Another live patient demonstration addressing carpal tunnel syndrome by MSKUS-guided median nerve hydrodissection was performed, thereafter. In both cases, attendees were able to witness the history taking, conduct physical examination, correlate the MSKUS scanning findings, and finally, apply MSKUS-guided interventional regenerative treatment.  There were break-out sessions where faculty members, including Edgar David Bionat, MD, FPARM, RMSK and Jan Tyrone Cabrera, MD, FPARM, RMSK, and attendees with a ratio of 1:4, were given time to scan using the described MSKUS scanning protocol.
This post-conference skills laboratory workshop was made possible through a collaboration with Sonoscape Ultrasound APEC and SA-US division. The company provided technical support and showcased their P80 machine among five others, which had Artificial Intelligence functions. This top-of-the-line unit has the capacity to connect the MSKUS monitor wirelessly to the LED screen or projector, which makes it ideal for teaching and instructional purposes for small to large audiences. It also features camera capacity to project the hand position of the scanner real-time, a feature which is best for teaching one-on-one, with a mentor even from remote on-line settings.
For regenerative procedures, JCS Pharmaceuticals Inc. providers of Xerthra and Biolevox collaborated with PARM in providing orthobiologics used for live patient demonstration. The platelet rich plasma (PRP) kits ensure the correct yield of platelet which is crucial for academic purposes and prognosticating the outcome of the clinical cases.

The 6-hour skills laboratory workshop culminated with a lunch buffet at the Food Exchange, Novotel Manila for all attendees and faculty. This is a high-yield activity which embodies the PARM spirit of quality commitment to its members, strengthening the continuing medical education of each member, and cementing its role in leading MSKUS-guided interventional regenerative treatment. PARM commits itself to the service of Filipino physiatrists through continuing medical education platforms because “You only see what you know.” When the diagnosis stares you straight in the eye, and it is something unfamiliar, it is an opportunity missed. Missing this opportunity is also amiss to the untrained eye. But when black, white, and the shades of gray make sense, it is every physiatrist’s canvass waiting to be turned into a masterpiece. Point of care ultrasound is life-changing be it for the patients or the physiatrists. But only to one who sees and knows what it is that it is.
